Diff to previous 1.2

Intermediate schema clean-up for RefDB imported data.
Implements most agreed schema changes, except not those which
would have required significant modifications to the provenance
tracking system.

Store object history in separate table.
Make not-understood columns nullable, and don't fill them.
Add key for event collection parentage type.

Diff to selected 1.3

Move run-related stuff to separate file.
Move parameter set on app_config, make it a string for now,
and make it part of the key on app_config.
